Client intake form template for photographers

Whether it's a portrait session or a full-day event, a clear intake form prevents the two things that hurt most: missed must-have shots and mismatched style expectations. Use these questions before you confirm the booking.

1. What are your names and the best contact details?
2. What type of session is it: wedding, portrait, family, event, commercial?
3. What is the date, start time and approximate end time?
4. Where will it take place? Please list all locations and addresses.
5. How many people will be photographed?
6. Which shots are absolute must-haves?
7. Share 3–5 photos (mine or others') whose style you love.
8. Are there venue restrictions I should know about (flash, areas, timing)?
9. Do you need a second shooter?
10. How many final edited photos do you expect, and by when?
11. Do you need prints or an album?
12. What is your budget range?
13. Who is the emergency contact for the day?
14. How did you hear about me?

How to use this intake form

• Ask for example photos — taste is easier to show than describe.
• Always collect a day-of emergency contact.
• Confirm delivery expectations (count, timing, format) in writing.
